What would you like to do with this file? Choice 1: If you only want to print a copy of the form without saving the file to your hard drive select Run this program from current location. You may
see a message that states Do you want to install and run form from www.centralmadison.com? Click Yes
and the form will appear on the screen and be ready to print. Choice 2: If you want to save the form for future use select Save this program to disk and click OK. A new window will pop up giving
you the opportunity to select the location on your hard drive where you want to save the file. (Many people like to save downloaded documents and programs in a file called downloads). Make a note of the file
name so that you can find it later on. You will see a message that says Download complete. At this point you can select Open
(to look at the form right away) or close the window if you are not ready to view or print the form. Netscape Instructions: Netscape will automatically download the Form to your hard drive. You will
